This prospective study assessed the feasibility and diagnostic value of a one-stop multienergy pancreatic perfusion CT (VPCT) protocol using photon-counting detector CT (PCCT) in 65 patients with histologically confirmed pancreatic ductal adenocarcinoma (PDAC). The protocol provided high-quality morphological and functional imaging with a mean radiation dose of 16.4 ± 4.5 mSv. Virtual monoenergetic images at 55 keV demonstrated the highest lesion contrast. Perfusion parameters differed significantly between tumor and normal tissue and correlated with tumor differentiation. This PCCT-based protocol supports a thorough, low-dose evaluation of PDAC, offering potential benefits for diagnostic accuracy and treatment planning.
