    1. Patient Presentation

    • A 76-year-old man with a past medical history of high blood pressure, diabetes, and chronic renal failure reported falling asleep at 10:00 pm the previous night. At 4:00 am he experienced involuntary movements and detachment from reality.
    • During ambulance transport, he suffered a generalized epileptic seizure.
    • Upon arrival at the emergency room at 7:00 am, he was alert but aphasic and showed right-sided motor deficit.
    • The mid-time point was estimated to be 2:00 am, corresponding to 5 hours from the onset of symptoms.
    • His score on the National Institutes of Health Stroke Scale (NIHSS) was 18.
    • A CT scan was performed at 7:35 am to assess for acute cerebral ischemic stroke.
