    1. Patient presentation

    • An 88-year-old man presented to the pneumology department with fever, chest pain, and progressive dyspnea and weakness over the past several weeks.
    • The patient reported having recurrent episodes of pneumonia in the previous months.
    • Chest radiography revealed extensive opacification of the left lung, suggesting lung cancer.
    • Contrast-enhanced MDCT of the chest, abdomen and pelvis was performed for further investigation.
