    1. Patient presentation

    • A 28-year-old man was admitted for atrial fibrillation.
    • A continuous murmur was heard in the left subclavian area.
    • Echocardiography showed dilatation and global hypokinesia of the left ventricle and a large
      dilatation of the left main coronary artery.

    • Cardiac CT was performed for further evaluation.
