A single-center retrospective study considered 66 patients diagnosed with colorectal cancer (CRC) and 42 controls without colonic disease. Conventional contrast-enhanced CT images and virtual monoenergetic image (VMI) reconstructions (energy levels between 40 and 100 keV, VMI40-100) were obtained from the late arterial phase of dual-layer dual-energy CT. Signal-to-noise ratio was evaluated to select the best VMI reconstruction, and the diagnostic performance of conventional CT images and VMI40 were compared. At quantitative image analysis, the best signal-to-noise and contrast-to-noise ratios were obtained with VMI40. In addition, the use of VMI40 proved to have superior diagnostic performance to conventional CT for the detection of CRC.
