This exploratory retrospective study evaluated the potential of multi-spectral CT (MSCT) for improving the detection of early gastric cancer (EGC) by addressing the limitations of conventional CT while offering a noninvasive alternative to endoscopy. Data from 141 patients were reviewed to identify imaging features associated with EGC. Quantitative assessment revealed that both lesion attenuation values and effective atomic number were significantly higher in tumoral tissue compared with normal gastric mucosa, in both the arterial and venous phases. Although the diagnostic performance of individual imaging parameters was limited, a multivariate discrimination model developed using LASSO regression achieved excellent diagnostic accuracy, with area under the curve values exceeding 0.90 in both training and validation datasets. Overall, these findings suggest that MSCT, especially when combining multiparametric analysis, may enhance the noninvasive detection of EGC and support more accurate diagnostic stratification.
