Higher iodine concentration agents, such as Iomeprol 400, improve visualization of the deep inferior epigastric perforators (DIEP) on preoperative CT angiography before DIEP flap surgery for breast reconstruction

A retrospective study evaluated the performance of the high-concentration contrast agent Iomeprol 400 compared with Ultravist 370 and Omnipaque 350 in visualizing the deep inferior epigastric perforators (DIEPs) prior to DIEP flap procedure for breast reconstruction. Forty women undergoing preoperative CT angiography were examined with Iomeprol 400 and 40 with either of the two lower-concentration agents. Iomeprol 400 provided significantly superior arterial enhancement of both proximal and distal segments of the deep inferior epigastric arteries and DIEPs (P<0.05), resulting in higher imaging clarity for the subsequent DIEP flap procedures.
